What Is HIP's Case Management Program?

HIP's Case Management Program is a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates options and services to meet an individual's health care needs. This is done through communication and by exploring available community resources to promote quality and cost-effective outcomes. Your PCP (Primary Care Physician) determines your care needs. Case managers and social workers coordinate these health care needs. They also educate members regarding their health conditions and serve as liaisons between members, all care givers and community agencies. For many health conditions, well-coordinated care and support are necessary to achieve the best possible outcome. That is one of the advantages of being a HIP member. If you have a condition that calls for a higher level of care coordination, HIP has case managers to help you get the right care in the right setting.
