Asthma
HIP is pleased to offer our Positive Actions Toward Health (PATH) Asthma Program at no cost to eligible members. Once enrolled, you will learn how to better control your asthma.
Participants in our PATH Asthma Program may receive:
- A welcome telephone call and a personalized health assessment from a professional asthma Health Coach.
- Educational mailings that include the latest information on asthma medications, treatments and more.
- Follow-up calls from and unlimited telephone access to a professional asthma Health Coach who can provide answers to questions about your condition, personalized support, education, and assistance with obtaining services.

Congestive Heart Failure (CHF)
HIP offers an innovative program for our members with Congestive Heart Failure (CHF). Now, you can take an active role in monitoring your condition through the use of remote technology.
The program provides a specially designed electronic scale with which to weigh yourself twice a day—right in the comfort of your own home. Answer a series of questions asked by an interactive communication device (in English or Spanish) that plugs into your home phone line. Your information will then be transmitted to specialized nurses who can alert your doctor to any changes in your condition.
This simple process, which takes less than three minutes to complete, enables your doctor to immediately address changes in your health.

Diabetes
If you have diabetes, you can register for HIP's diabetes program. Enrolled members will receive tools and access to educational classes with the goal of helping to better monitor and manage diabetes. HIP will also send a personalized annual report to both you and your doctor summarizing test results from the past year and advising which exams may be overdue.
It is important to learn all you can about diabetes and how you can better control it. Ask your doctor to contact us and have you enrolled in HIP's free diabetes program.

End Stage Renal Disease (ESRD)
Our End Stage Renal Disease (ESRD) Program, available to eligible members, offers you the opportunity to attend an educational class where you can learn more about your condition. You will also receive a variety of informational materials designed to enhance your quality of life and educate you on treatment options such as kidney transplantation.
Members enrolled in this program have access to a team of nurses who are available to help you prepare for dialysis treatment, answer questions, and provide assistance during a health crisis or emergency. Our nurses work directly with your doctor and other health professionals to make sure you are getting the services you need.
