Services
Coordinated Care
A program specialist helps coordinate your care to ensure that all your needs for an effective, comprehensive care plan are being met.
Compassionate Support
We are committed to preserving your quality of life by ensuring that you are aware of support services available to you and your family. We will advocate on your behalf with physicians and health care providers to ensure that your and your family wishes are respected by everyone involved in your care.
Comprehensive Education
A program specialist explains all your care options, including hospice and palliative care. We provide educational materials and resources specifically designed for dealing with issues that arise at a time of advanced illness. There are many care options available. You don't have to make these choices alone.
Treatment Evaluation
We offer assistance in determining what kind of treatment is appropriate for you and what your choices are. We will collaborate with you and your health care practitioners to ensure you have all the medical information needed to make decisions about your treatment.
Case Management
The Plan's case managers can work with you and your support person(s) to coordinate all aspects of care, including access to health care or social services.
