- All Child Health Plus, Medicaid and Family Health Plus members are assigned to a primary care dentist listed in this directory as of their effective date of enrollment in HIP.
- Dentists listed in this directory represent all participating dentists that render dental services to Child Health Plus, Medicaid and Family Health Plus members.
HIP arranges for the delivery of oral health care services in accordance with your benefit coverage.. HIP does not directly provide these services. Rather, these services are provided by Independent Contractors. The dental providers listed in this directory that deliver oral health care services are not the employees or agents of HIP. HIP shall not be liable for any negligent act or omission by any of the providers listed in this directory, or any of their employees or agents, who may, from time to time provide dental services to you. HIP expressly disclaims any agency relationship, actual or implied, with any health care provider. Any decisions made by HIP concerning appropriateness of setting, whether any service or supply is dentally necessary, pursuant to your health care coverage shall be deemed to be made solely for the purposes of determining whether benefits are due under package of covered benefit, and not for the purposes of recommending any dental treatment or non-treatment. HIP does not exercise any control or direction over the medical/dental judgment or clinical decisions of any dental provider listed in this directory and does not interfere with the dentist patient relationship between you and any dental provider.
