NYC School Admission Requirements *

Admission requirements help ensurethat children receive basic medical preventive care.  The New York City Department of Health and Mental Hygiene requires that new students-children entering pre-kindergarten though grade 12 of any public or private day-care center or school in New York City for the first time-must show proof of having received a complete medical evaluation.  (Children who transfer from one school to another in New York City are not considered "new" students.) Immunization requirements appear below. This year, there is a new requirement:  In September 2000, all children entering the 7th grade (not just new students) must have the full series of hepatitis B vaccine to comply with New York State law.  Any student entering an ungraded class who is 12 years old (or who will be 12 years old by December 31, 2000) should also be vaccinated. In addition:

  • All children (not just new students) should have an annual physical examination until the 6th birthday that includes:  height; weight; blood pressure; medical history; a nutritional evaluation; a developmental assessment; vision, hearing, dental, and anemia screening, and have a risk assessment for lead exposure.  A direct blood lead test is required at ages 1 and 2 years, and whenever the annual risk assessment determines high risk.  Until the 6th birthday, all children need documentation of having received a direct blood lead test at least once.

  • A Mantoux tuberculin skin test is required for new students entering secondary schools(intermediate, junior high, middle schools and high schools).

*Production shortages of some vaccinations may require modification of the schedule until production is back to normal. This information is given to your child's physician and he/she will advise you of these changes.
